2008 Driver Profile Questionnaire

The online form is asking for information that the ASAMT feels that the fans, media, and sponsors would like to know about each competitor. The idea is to allow the fans to get to know the drivers and their teams prior to coming to or after they saw an ASAMT event.

Each team is encouraged to take a few minutes and fill out the form as soon as possible. The information will be used for their profile on the ASAMT’s website, media guides, and the official program along with assisting the track announcer(s).

In keeping with ASA Racing Tradition, please list the hometown (location of birth) for the driver of the race team.
Please also list if race team is driver owned
Assigned by the ASA Midwest Tour
Please list your sponsors in order of importance.
Please include as much information as possible. The more thorough you are, the better the ASA Midwest Tour can hype your accomplishments.
Please include a phone number or email for Media to contact you for press and/or profiles.
